Qualifications

• Bachelor’s degree in social work or related field required.
• Licensed professional required.
• Must be able to write, speak, facilitate, and translate in both English and Spanish.
• Excellent communication and interpersonal skills.
• Case Management and Outreach experience required.
• Electronic Medical Records experience preferred.

Responsibilities

• Provide comprehensive case management including intake assessment, benefit assessment, financial goal setting, and financial literacy education.
• Serve as a point of contact for hospitals, health care providers, emergency responders, and social services.
• Collaborate with internal and external resources to ensure referrals and follow-up services.
• Coordinate transportation for participants within Illinois.
• Document all participant interactions and maintain program records.
