Job summary

Duane Dean Behavioral Health Center in Illinois seeks an IWC Case Manager to deliver comprehensive case management and education through the Illinois Welcoming Center program. Responsibilities

  • Provide comprehensive case management including intake, benefit assessment, goal setting, money management and financial literacy training.
  • Engage as a community ambassador; participate in events, schools, meetings and church gatherings.
  • Document all contacts and referrals accurately in the participant files.
  • Coordinate referrals with staff and ensure follow-up services are provided as needed.
  • Link families to community resources and document referral outcomes; weekly case plan development and advocacy.
  • Participate in staff meetings and trainings; manage urgent care needs and data reporting.

Job Location: Kankakee

Salary Range: $40,000-$45,000

Duane Deane - Who we are:

For over 50 years, Duane Dean Behavioral Health Center has provided substance use treatment, intervention and prevention services to adults who reside in Kankakee, Iroquois counties and throughout east-central Illinois. Working in collaboration with our management organization, Rincon Family Services, since 2017, we have expanded our services to include violence intervention, workforce development, mental health and welcome center services.

Position Description – What the Role Entails:

The Case Manager will identify individuals in the community who would benefit from education or services offered by the Illinois Welcoming Center (IWC). The case manager will function as a conduit to engage, educate, and promote the IWC program to community members and stakeholders.

Areas of Accountability - What you will be doing:
The Knowledge and Expertise that Matters Most - What you bring to the Role:
  • Provide comprehensive and culturally sensitive case management services to all clients, including intake assessment, benefit assessment, financial goal setting, educational goal setting, money management, and financial literacy training, to name a few.
  • Be a community ambassador for the program participating in community events, schools, meetings, church gatherings.
  • Document accurately, neatly, and consistently every and any contact with or on behalf of a participant in the participant’s designated file, as required by funding source.
  • Work in conjunction with agency staff to ensure that appropriate referrals are made and follow-up services are provided as needed.
  • Connect families with needed and available community resources. Follow up and document with referrals agencies to ensure the use/success of the referral.
  • Weekly case plan development, progress monitoring, advocacy, and referrals.
  • Provide linkages for clients to community-based services as needed; stay abreast of staff changes and referral procedures at partner agencies.
  • Participate in staff meetings, staff training, and other duties as assigned by the program supervisor.
  • Assess and provide urgent care services, items and resources to IWC clients.
  • Become familiarized with internal and external reporting and case-management programs and procedures.
  • All other duties as assigned.
OTHER REQUIRED ABILITIES:
  • Bilingual Spanish/English and Bicultural applicants strongly preferred.
  • Ability to work effectively with peers, individual participants, and groups.
  • Desire to learn new skills.
  • Knowledge of community and social service agencies.
  • Ability to maintain accurate records and necessary paperwork, as well as data entry – Currently Sales Force and Internal Trackers.

Position Qualifications – What is required to Perform the Role:
  • High School Diploma or GED; enrolled in an Associates/Bachelor’s program.
  • At least 1 year of previous case management experience is preferred.
  • Knowledge and understanding of the local community and its stakeholders.
  • Have a vehicle, valid driver’s license, vehicle insurance, and a safe driving record.
