Qualifications

• Zero to one year full-time experience working with individuals receiving mental health or related services.
• Ability to provide nurturing attention and care to individuals with disabilities.
• Ability to communicate with verbal and nonverbal individuals.

Responsibilities

• Teaches, models, and coaches activities of daily living.
• Provides transportation and ensures safety practices.
• Collaborates with community systems and care teams.
• Documents services and maintains confidentiality.
• Attends staff meetings and trainings.
• Assists with discharge planning as needed.
