Job summary

Seven Counties Services is seeking a part-time Direct Support professional to teach daily living activities, provide psychoeducational support, and assist individuals with disabilities in community activities. You will collaborate with schools, families, and service providers to ensure safe, coordinated care and smooth transitions between settings.

The role requires the ability to transport clients, support personal care tasks, and document care activities in accordance with program protocols

Qualifications

• 0–1 year full-time experience with individuals receiving mental health, substance abuse, intellectual disability, or physical health services.
• Ability to provide nurturing care to individuals with disabilities.
• Ability to communicate with verbal and nonverbal individuals.
• Good interpersonal skills.
• Teaching skills.
• Patience and empathy.
• Maturity and judgment.

Responsibilities

• Teaches, models, and coaches activities of daily living (ADL) and monitors performance.
• Teaches psychoeducation on recovery and related topics.
• Plans and facilitates community-based activities and access to resources.
• Maintains safe, therapeutic environment and uses de-escalation techniques.
• Acts as an ambassador for Seven Counties Services and educates the community about services.
• Collaborates with other systems (schools, landlords, transportation, employers).
• Provides transportation and facilitates use of public transit.
• Provides personal care as needed (hygiene, toileting, meals).
• Administers First Aid and CPR as needed.
• Supervises self-administered medication per program protocols.
• Identifies allergies and avoids exposure.
• Documents services with timely forms.
• Maintains confidentiality of health information and adheres to policies.
