Job summary

VNS Health Community Center seeks a dedicated individual to assist in coordinating activities, acting as a liaison between external customers, patients, families and VNS Health, and promoting the Center's services.

The role includes educating community members about VNS Health lines of business, promoting services (CHHA, Hospice, Personal Care, Health Plan), handling referrals, and maintaining accurate records to ensure smooth transitions and high customer satisfaction.

Qualifications

• High School Diploma or GED required.
• Two years of customer service experience required.
• Proficient in speaking, reading, and writing Chinese.
• Fluency in other dialect is preferred.
• Experience in a healthcare organization or in social services preferred.
• Demonstrated knowledge of Microsoft Windows preferred.

Responsibilities

• Coordinate activities for the VNS Health Community Center.
• Liaise between external customers, patients, families and VNS Health to promote services.
• Educate community members about VNS Health lines of business.
• Promote VNS Health services including CHHA, Hospice, Personal Care, and Health Plan.
• Process referrals and ensure smooth intake and enrollment workflows.
• Recruit and retain members and support business growth.
• Maintain reports on referrals and admissions with accuracy and efficiency.
