Job summary

Visions Case Management NM is seeking a Service Provider to strengthen communities in New Mexico by enhancing the lives of clients served. The role includes facilitating monthly contact, drafting and submitting annual plans/budgets, and maintaining up-to-date client case files.

The position requires a four-year degree in Nursing, Psychology, or Social Work with related experience, or an equivalent degree plus years of experience.

Qualifications

• Four year degree in nursing, psychology, or social work with related experience.
• Willingness to travel within the designated region of the state.

Responsibilities

• Facilitate monthly contact with consumers and submit related documentation in a timely manner.
• Maintain complete and current client case files.
• Update internal tracking systems (Google Sheets) twice weekly and keep information current.
• Participate in monthly staff meetings and meet with regional lead weekly or program manager monthly for supervision.
• Respond to internal and external communications within 24 hours.
