Job summary

Visions Case Management & Consulting is seeking a Service Provider to strengthen communities in New Mexico by improving the lives of clients and collaborating with teams. The role emphasizes documentation, program planning, and responsive communication.

You will coordinate with regional leads, manage client files, and participate in regular supervision and training to ensure high-quality service delivery. Hybrid work and a flexible schedule are noted as perks.

Qualifications

• Four-year degree in Nursing, Psychology, or Social Work with 1 year of related experience.
• Any four-year degree and 6 years related experience.
• Travel within the state of New Mexico.

Responsibilities

• Facilitate monthly contact with consumers and submit related documentation promptly.
• Draft and submit annual plans and budgets per program timelines.
• Maintain complete and current client case file.
• Utilize and update internal tracking systems (Google Sheets) 2x weekly.
• Participate in monthly staff meetings and meet with regional lead weekly or program manager monthly for supervision and training.
• Respond to internal and external communications within 24 hours.

As a Service Provider at Visions Case Management & Consulting, you will be strengthening the communities of New Mexico by enhancing the lives of those we serve and work with. Will also be a source of information, to foster a supportive environment, to deliver exceptional services, and to empower everyone you interact with.

Responsibilities:

You will have the following responsibilities:

• Facilitate monthly contact with consumers and submit related documentation in a timely manner.
• Drafting and submitting annual plans/budgets according to program timelines and revise as needed.
• Maintain complete and current client case file.
• Utilize and update internal tracking systems 2 x weekly and ensure information is current and complete. Specifically google sheets or spreadsheets in general
• Participate in monthly staff meetings and either meet with your regional lead weekly or program manager monthly for supervision and training.
• Respond to internal and external communications within 24 hours.

Required Qualifications:

• Four Year Degree in Nursing, Psychology, Social Work and 1 year of related experience
• Any Four Year Degree and 6 years of related experience
• Travel within an entire state of New Mexico
