Job summary

Responsibilities

• Coordinate care with physicians, nursing staff, case managers and social workers to ensure timely discharge and optimal outcomes.
• Conduct psychosocial assessments and plan interventions to address patient needs.
• Develop and maintain discharge planning to expedite timely transitions to appropriate levels of care.
• Document assessment data and patient progress and communicate with families and care teams.
• Participate in patient safety and quality improvement initiatives.
