Job summary

Wellnest is seeking a Case Manager to join a multi-disciplinary team serving children, youth, and families in Los Angeles. You will help develop, implement, and monitor treatment plans, coordinate with therapists, and connect clients to health insurance, housing, and child care resources.

Responsibilities include conducting needs assessments, providing rehabilitation services, completing timely documentation, and advocating for families in the community.

Qualifications

• Bachelor's degree from an accredited college or university in social work, human services or a related field.
• One to two years experience providing case management services to low-income individuals in a community setting with at-risk populations.

Responsibilities

• Works with therapist to conduct comprehensive needs assessments with children, youth, adults, and families to assist them in obtaining needed resources.
• Provides individual and group rehabilitation services under the Department of Mental Health contract or other agency funding.
• Meets agency's productivity standards for client service hours.
• Completes and submits progress notes within 24 hours using the agency's EHR system and discusses insurance/Medi‑Cal information with relevant teams.
• Provides guidance to parents/caretakers on managing child’s behaviors and familial stressors.
• Assists families in identifying internal resources to address multiple stressors.
• Develops client rehabilitation services to improve daily functioning in home, school, and community.
• Provides interagency consultation, communication, coordination and referrals.
• Attends community meetings and represents the agency at resource fairs.
• Follows up with families that drop out of treatment.
• Works in communities, homes, schools and other field-based settings.
