Qualifications

• Education: Associate’s Degree in Rehabilitation, Social work, or related field preferred and/or 1 year related work experience.
• Skill Requirement: Three months OJT required for efficient job performance.
• Job Requirements: Excellent organizational skills and ability to prioritize tasks; flexibility due to non-set schedule.

Responsibilities

• Transport clients to appointments as necessary.
• Prepare daily contact sheets and monthly progress notes for each client.
• Serve as liaison between clients and mental health professionals, family, therapist, and community agencies.
• Perform crisis intervention and assistance as necessary.
• Attend staff meetings to address concerns and supervision issues.
• Hold meetings with clients to address concerns and coordinate appointments.
• Develop Individualized Rehabilitation Plans and conduct planning meetings.
• Seek housing placements for clients and monitor housing adequacy.
• Monitor entitlement access and client health and medications.
