Qualifications

• Bachelor's/Master's degree in psychology, social work or related field plus 2 years of relevant experience.
• Must have and maintain a valid driver's license with an acceptable driving record.

Responsibilities

• Care Coordination: Oversee and manage client care plans, ensuring timely access to services and resources.
• Collaboration: Work with providers, clients, and families to develop comprehensive care plans.
• Monitoring Progress: Assess and monitor client progress and adjust care plans as needed.
• Client Advocacy: Advocate for clients' needs to ensure high-quality care.
• Documentation: Maintain case documentation and client records per regulatory standards.
