Acenda Integrated Health seeks a Care Manager Hourly to support clients in Cape May County needing mental health services. You will work with a team to help clients obtain treatment, maintain care, and navigate services toward wellness.

Qualifikationen

• Bachelor’s Degree with 2 years’ experience in the human services, psychology or related field.
• Resident of the geographical areas identified or with relevant knowledge of counties served.
• Must have a valid driver’s license, use of an insured vehicle, and acceptable driving record.
• Must be sensitive, flexible and responsive to diversity and individual needs.
• Professionalism and commitment to excellence in care.

Aufgaben

• Serve as the contact point, advocate and informational resource for clients and care teams.
• Partner with clients to monitor adherence to treatment plans and evaluate progress.
• Facilitate client linkages, referrals and access to services per the treatment plan.
• Educate patients and families about community resources and connect to services.
• Conduct clinical follow up after inpatient or emergency care and coordinate transitions.
• Cultivate relationships with external providers and supports.
• Complete and submit accurate reports and chart documentation timely.
• Ensure quality and accuracy of written and verbal communications.
• Prepare reports and data for dashboards, market research or program development.
• Support Quality/Compliance/Risk functions as required.
