Zusammenfassung

Acenda Integrated Health is seeking a Care Manager (Hourly) in Egg Harbor Township to provide case management and in-home therapeutic sessions for families facing housing instability. You will partner with clients, track progress, link to services, and document outcomes consistent with program guidelines.

Responsibilities include coordinating with care teams, maintaining a ~10–12 family caseload, educating families about resources, and fostering external partnerships to improve stability.

Qualifikationen

• Bachelor’s level practitioner with at least 2 years of experience in social service-related field preferred.
• Must have and maintain a valid driver’s license with an acceptable driving record
• Must be able to exercise independent thinking and good judgment under all circumstances.
• Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.

Aufgaben

• Serve as the contact point, advocate and informational resource for clients, care team, family/caregivers and community resources regarding client's service plan
• Partner with clients to monitor adherence to their service plan, evaluate effectiveness, monitor progress in a timely manner and facilitate changes as required.
• Facilitate client linkages, referrals and access to appropriate treatment, community supports and medical or specialty providers according to the service plan
• Maintain an active caseload of approximately 10-12 families, providing in person in home therapeutic sessions to address treatment plan goals
• Educate patient and family/caregiver(s) about relevant community resources
• Cultivate and maintain relationships with external community providers and supports.
• Complete and submit accurate reports and chart documentation in a complete and timely manner according to program guidelines and established procedures.
• Ensures quality and accuracy of written and verbal communications.
• Support the Quality/Compliance/Risk function as required.
• Performs other related duties as assigned.
