Job summary

Chapman Partnership is seeking a Community Case Manager in Homestead/Miami, FL. This full-time, hourly role supports families by coordinating services, conducting home visits, and guiding clients toward stable housing and access to benefits.

You will document progress, participate in planning, and collaborate with housing staff to ensure timely assistance. A Florida driver's license is required.

Qualifications

• Bachelor's degree in social/human services or related field.
• Minimum of one year of practical case management experience in a social services setting.
• Valid Florida driver's license with a clean driving record.

Responsibilities

• Assess families within 72 hours of admission to determine services and develop individualized plans.
• Provide community-based support to help families stabilize and transition to independence.
• Maintain up-to-date case notes, goal plans and records; document activities and reports for Chapman Partnership.
• Participate in case conferences to support discharge and outplacement planning for housing and employment.
• Collaborate with Housing Specialists to monitor clients for up to a year through outreach and visits.
