Job summary

Chapman Partnership is seeking a Community Case Manager to provide holistic, community-based support and ensure safe housing transitions for families in Miami‑Dade County. You will conduct timely assessments, complete documentation, and coordinate with Housing Specialists to help clients regain stability.

Responsibilities include developing individualized plans, maintaining records, attending case conferences, and guiding clients toward available resources and benefits, with a focus on durable

Qualifications

Bachelor's degree in social services or related field or equivalent experience.
Minimum of one year of practical case management experience; four years can substitute for education.
Valid Florida driver license and clean driving record.

Responsibilities

Assess families within 72 hours of admission to determine services and plan goals.
Provide community-based support to help families stabilize and transition to independent living.
Maintain case notes, goal plans, and required reports for Chapman Partnership.
Participate in case conferences to support discharge and housing/employment plans.
Monitor out-placed clients for up to a year through calls and home visits.
Refer families for ongoing planning and resources, including DCF benefits.
