Job summary

Resources for Human Development (RHD) is seeking a dedicated Case Manager in Norristown, PA for the Positive Resolutions Program. You will coordinate recovery services, connect participants with resources, and advocate for needs within a multidisciplinary team.

This full-time role involves community-based work across Montgomery County, with a focus on helping individuals achieve goals in their treatment and recovery plans. DailyPay is available; compensation is $22.00 per hour.

Qualifications

• Two (2) years of experience providing support to individuals in behavioral health, addiction recovery, unhoused services, or IDD/BHH programs
• Intermediate computer skills
• Strong communication and organizational abilities
• Ability to work independently and in community-based settings
• Bachelor's Degree in Human Services or a related field (preferred)
• One year of case management experience (preferred)
• Knowledge of behavioral health resources and community support systems (preferred)

Responsibilities

• Work as part of a multidisciplinary team to provide holistic, person-centered services.
• Assist individuals in developing and achieving recovery goals.
• Coordinate services and connect participants with community resources and supports.
• Build strong relationships with community agencies and service providers.
• Advocate for participants to ensure their needs are met and barriers are addressed.
• Provide community-based case management services throughout the service area.
• Transport participants to appointments and community resources when needed.
• Complete assessments, service plans, reviews, and required documentation.
• Maintain accurate and timely records in accordance with agency and funder requirements.
• Collaborate with clinical staff, medical providers, and community partners to support positive outcomes.
