Job summary

Renaissance Social Services, Inc. is seeking an experienced SUPR/Outreach Case Manager to provide intensive, community-based case management for adults living in scattered-site housing across Chicago.

The role supports recovery goals within a harm reduction framework, meeting participants where they are and coordinating services with partners. Qualifications

• Must meet Illinois criteria for a Mental Health Professional (MHP) via one of the pathways.
• A minimum of 3–5 years of clinical experience, preferably in case management.
• Experience with chronic mental illness and substance use disorders, with a firm commitment to Harm Reduction principles.

Responsibilities

• Provide intensive, community-based case management to participants in scattered-site housing.
• Support participants' substance use recovery goals within a harm reduction framework, at a pace set by the individual.
• Assess participant needs and barriers to services, and develop and carry out individualized service plans.
• Connect participants to community resources through information, referral, linkage, and advocacy, and coordinate services with other providers.
• Provide crisis intervention and de-escalation when participants are in crisis, in consultation with a QMHP.
