Renaissance Social Services, Inc. in Chicago is seeking an experienced SUPR Outreach Case Manager to provide community-based case management for adults in scattered-site housing.

You will support recovery goals within a harm-reduction framework and help participants maintain housing, build skills, and access resources. Travel across Chicago is required; we offer full-time salaried employment with benefits, including health coverage, a Simple IRA, paid holidays, and mental health resources.

Qualifikationen

• Must meet the State of Illinois criteria for a Mental Health Professional via one of the listed pathways.
• Minimum 3–5 years of clinical experience, preferably in case management or direct community-based service.
• Experience with chronic mental illness and substance use disorders, with Harm Reduction principles.

Aufgaben

• Provide intensive, community-based case management in scattered-site housing through home visits and field outreach.
• Support participants' recovery goals within a harm reduction framework, at a pace set by the individual.
• Connect participants to community resources through information, referral, linkage, and advocacy, and coordinate services with other providers.
• Provide crisis intervention and de-escalation when participants are in crisis, in consultation with a QMHP, and coordinate follow-up care.
• Travel throughout Chicago using a personal vehicle to deliver services in participants' homes and community settings.
