Job summary

Renaissance Social Services, Inc. in Chicago seeks a Family and Senior (FSP) Clinical Case Manager to provide intensive clinical outreach and supportive services to participants with mental health and substance use diagnoses in scattered-site units across the city.

You will address psychosocial, health, financial, housing, and recreational needs. The role requires a minimum 1 year of relevant experience, a State MHP criteria alignment, strong commitment to Harm Reduction, and a valid driver's

Qualifications

• Must meet State of Illinois Mental Health Professional criteria.
• Minimum of 1 year working with individuals experiencing chronic mental illness and substance use disorders.
• Valid driver’s license and daily access to a reliable vehicle for community-based work.
• Proof of active automobile insurance with minimum liability limits.

Responsibilities

• Provide intensive mental health case management services in the community.
• Identify client needs and assist with effective problem solving to address issues.
• Provide treatment planning, coordination, and implementation of services needed.
• Coordinate the provision of services with other providers and community agencies.
