JOB SUMMARY

The Family and Senior (FSP) Clinical Case Manager provides intensive clinical outreach supportive services to program participants with mental health and substance use diagnoses living in scattered site units across the city of Chicago. Services include attending to the psychosocial, health, financial, recreational, nutritional, and housing needs of the clients.

ESSENTIAL FUNCTIONS
  • Provide intensive mental health case management services in the community, including community building and social activity planning and implementation.
  • Identify client and resident needs, including barriers to receiving appropriate services and assisting with effective problem solving to address issues.
  • Provide treatment planning, coordination, and implementation of services needed.
  • Provide information, linkage, referral and advocacy for clients and residents with agencies in the community, including crises intervention services when needed.
  • Work with clients and residents to increase their skills and income, improve their self‑determination, and maintain residential stability.
  • Maintain the confidentiality of client information in any written, oral, or electronic form including, but not limited to, health information, HIV/AIDS status, and disability information. Releasing such information only as needed and with the proper release from the client unless mandated by law.
  • Complete the submission of billing sheets in accordance with Rule 132 guidelines.
  • Report all incidents of abuse, and/or neglect as required by state law.
  • Maintain accurate and up‑to‑date records using HMIS and paper files on all clients and residents receiving services, including maintaining records necessary to fulfill grant requirements and third party payers.
  • Coordinate the provision of appropriate services from other providers from the community at buildings at which clients and residents are living.
  • Work cooperatively with building management to ensure residential stability of clients and residents.
  • Identify and develop training opportunities for clients and residents.
  • Complete required documentation for Medicaid billing, HMIS and compliance in a timely manner.
QUALIFICATIONS - MINIMUM REQUIREMENTS
  • Must meet State of Illinois Mental Health Professional (MHP) criteria through one of the following pathways:
    • Bachelor’s degree in a human services field (e.g., Social Work, Psychology, Counseling), OR
    • Bachelor’s degree in an unrelated field plus 2 years of documented clinical experience under the supervision of a QMHP, OR
    • High School Diploma or GED plus 5 years of documented clinical experience in mental health or human services.
  • Minimum of 1 year working with individuals experiencing chronic mental illness and substance use disorders.
  • Demonstrated commitment to Harm Reduction techniques and trauma‑informed care.
  • Valid driver’s license and daily access to a reliable vehicle for community‑based work.
  • Proof of active automobile insurance with minimum liability limits of 100/300/100.
  • Strong computer literacy, including experience with Microsoft Office and the ability to learn Electronic Health Record (EHR) systems for daily documentation.
PREFERRED QUALIFICATIONS
  • Active CRSS (Certified Recovery Support Specialist) or CPRP (Certified Psychiatric Rehabilitation Professional) designation.
  • Experience in crisis intervention, de‑escalation, and basic risk assessment.
  • Strict adherence to HIPAA confidentiality regulations and data privacy protocols to ensure the secure management of protected health information (PHI).
  • Exceptional written and verbal communication skills, with an emphasis on timely and accurate clinical note‑taking.
  • Experience working within a Community Mental Health Center (CMHC) or similar non‑profit setting.
