Hope Network is seeking an IDDT Case Manager to support Genesee Case Management in Flint, MI. This full-time, dayshift role requires community-based work, including home visits and care coordination to maximize recovery and independence.

Qualifications

• Bachelor’s degree in a human services field from an accredited institution.
• Experience working with dual diagnosis (substance abuse/mental health).
• Valid Michigan Driver’s License.
• Willingness to work in the community and perform home visits.

Responsibilities

• Work in the community performing home visits.
• Ensure consumer involvement in the PCP (Person-Centered Planning).
• Facilitate the implementation of Individual Plan of Service.
• Perform comprehensive, ongoing psychosocial assessments.
• Participate in legal proceedings including petitioning and testifying in clinical emergencies.
• Provide community-based services.
• Facilitate the coordination of consumer intakes, discharges, and transfers.
• Communicate with all service providers in Treatment Plan.
• Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social workers.
• Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.
• Provide referral to appropriate community resources.
