Job summary

AltaPointe Health Systems is seeking a dedicated Case Manager to support individuals in community-based settings across mental and physical health needs. You will travel to homes, schools, and other sites to monitor well-being and progress, empowering consumers and families with information and resources.

Responsibilities include assessing needs with the SUN-R, developing ISP goals, and coordinating services with treatment teams to secure least restrictive placements.

Qualifications

• Bachelor’s degree in a behavioral health or social services related discipline preferred.
• Must meet all driving requirements.
• Knowledge of adult behavioral health topics and community resources is expected.

Responsibilities

• Assess the consumer’s needs using the SUN-R and document goals in the ISP.
• Provide linkage to community resources and monitor progress.
• Advocate for consumers to secure appropriate services and supports.
• Collaborate with treatment teams and families to support least restrictive environment placement.
• Provide outreach and training of basic living skills to enhance quality of life.

Education
