Qualifications

• Knowledge of community resources and internal services; six-month learning window.

Responsibilities

• Maintains caseload effectively; processes referrals in a timely manner.
• Conducts a Functional Needs Assessment on each referral.
• Collaborates with Care Coordinator to address medical needs.
• Creates and maintains an ISP within 30 days of contact.
• Links to community resources and coordinates internal services.
• Keeps an itemized resource list and monitors ongoing need.
• Participates in Performance Improvement activities and follows policy.
