Job summary

Catholic Charities, Diocese of Cleveland is seeking a Case Manager at the OLA St Joseph Center in Cleveland, Ohio. The role provides case management and supportive services to help clients improve social, economic, cultural, or educational status and restore their capacity for social functioning.

Responsibilities include direct service in the office or field, service coordination, advocacy, and maintain comprehensive case records while collaborating with a multidisciplinary team.

Qualifications

• Bachelor’s degree and 1–2 years related professional experience.
• Competent oral, written and interpersonal communication skills.
• Well organized, detail oriented and ability to maintain confidentiality.
• Ability to work with diverse client population and staff at all levels.
• Ability to form good working and therapeutic relationships.
• Fingerprinting required for background check.

Responsibilities

• Provide direct service to clients in the office or in the field.
• Review and monitor services; reassess needs and initiate services.
• Assist clients in developing formal and informal community support.
• Provide advocacy on behalf of clients.
• Prepare and complete case recordings, admin reports and related paperwork.
• Attend clinical and administrative staff meetings and multi-disciplinary team meetings.
• Review treatment plans and progress with supervisor for continued support.
• Perform other duties as necessary to accomplish objectives.
