Job summary

Catholic Charities, Diocese of Cleveland is seeking a compassionate case manager to provide case management and supportive services to clients under supervision, helping them improve their social and economic status.

The role includes direct client service in the office or field, coordinating services, advocating for clients, maintaining confidential records, attending team meetings, and reviewing progress with a supervisor. This is a full-time, non-exempt position based in Cleveland, OH.

Qualifications

• Bachelor's Degree and 1–2 years related professional experience.
• Strong oral, written, and interpersonal communication skills.
• Organized, detail-oriented, and able to maintain confidentiality.
• Ability to work with diverse client populations and staff at all levels.
• Ability to form good working and therapeutic relationships.
• Fingerprinting/background check required.

Responsibilities

• Under direction, ensure that activities of this position and relevant programs align with the mission, vision, and values of Catholic Charities, Diocese of Cleveland.
• Provide clients direct service, either in the office or in the field.
• Review and monitor services, reassess needs, and initiate services which are responsive to the needs of clients; provide service coordination.
• Assist clients in developing formal and informal community supports.
• Provide advocacy on behalf of clients.
• Prepare and complete all required case recordings, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner.
• Attend clinical and administrative staff meetings, multi-discipline team meetings as required.
• Review treatment plans and progress of cases with supervisor for continued support, advice, and approval.
• Perform other duties as necessary to accomplish objectives.

Requirements

• Combination of education and experience normally represented by a Bachelor's Degree and one to two years related professional experience.
• Must have competent oral, written and interpersonal communication skills.
• Must be well organized, detail oriented and have the ability to maintain confidentiality.
• Ability to work with diverse client population, community organizations as well as staff at all levels.
• Must be able to form good working and therapeutic relationships.
• Final applicant is required to be fingerprinted to complete background check.
