Job summary

Holcomb-Behavioral-Health-Systems is seeking a full-time Case Manager to provide community-based behavioral health services. You will assess client needs, develop individualized plans, and coordinate with families and providers. The role requires crisis intervention skills, strong documentation, and a commitment to recovery-based care.

The position features a Monday–Friday schedule (9:00 am–5:00 pm) and pays $20+ per hour, with eligibility for benefits and on-call rotation responsibilities.

Qualifications

• Bachelor-level education in sociology, social work, psychology or related fields or RN qualification.
• RN qualification or equivalent clinical credential accepted.
• High school diploma with 12 college credits in social sciences plus 2 years front-line human services experience with direct client contact.
• High school diploma with 5 years mental health direct care experience in human services.

Responsibilities

• Carry a caseload of up to 30 clients and perform case management duties.
• Provide services based on Recovery Model principles to clients.
• Develop strength-based, measurable service plans with targeted outcomes.
• Assist clients in obtaining housing, food, healthcare, employment and social support.
• Coordinate care between providers and act as single point of contact for linked services.
• Provide crisis assessment and intervention when needed.
• Submit timely payroll and billing documentation and maintain records.
