Job summary

Holcomb Behavioral Health Systems is seeking a Case Manager to provide community-based services for individuals eligible for behavioral health services in Kent County. You will assess resources, develop recovery-focused plans, and support clients in securing housing, healthcare, and employment.

Schedule is Monday–Friday 9:00 am–5:00 pm with 24/7 on-call rotation. Salary offered is competitive with comprehensive benefits across a multi-state service network.

Qualifications

• A bachelor’s degree with major coursework in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education.
• Be a registered nurse.
• A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science, and 2 years of experience in public or private human services with 2 years in direct client contact.
• A high school diploma and 5 years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.

Responsibilities

• Carry a caseload of up to 30 clients.
• Perform case management duties for consumers involved in Behavioral Health Services.
• Provide services based on Recovery Model principles.
• Develop individualized, strength-based service plans that are measurable and lead toward targeted outcomes.
• Assist consumer service needs and assess appropriate mental health services.
• Assist consumers in obtaining and maintaining basic needs such as housing, food, healthcare, employment, and socialization.
• Assist consumers through assertive and creative efforts to gain needed resources and services identified in the service plan.
• Monitor the consumer’s participation in the recovery plan and support services.
• Provide crisis assessment and crisis intervention when necessary.
• Maintain professional relations with the consumer, families, payers, and other agencies.
• Coordinate communication between all service providers and support systems.
• Act as a single point of contact for health and social services linkages.
• Provide client service and documentation for re-authorizations timely.
• Submit payroll and billing documentation accurately and timely.
• Be available for an on-call rotation 24/7.

Education

Bachelor's degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education

Registered nurse
