Job summary

Ak-Chin Indian Community seeks a Case Manager to respond to community needs under the Health and Human Services Director. The role focuses on integrating best social work practices, with potential for night, weekend, and holiday shifts.

Qualifications

• Bachelor's degree in Social Work, Public Health, Psychology, Education or related field.
• Three years of professional level social work experience or equivalent.
• Arizona driver's license and ability to meet insurance requirements.
• Must pass fingerprint/background check and remain alcohol/drug free.
• Experience with Native American Communities or Tribes is ideal.

Responsibilities

• Provide intake services for referrals to General Assistance, Burial Assistance, Homemaker Services, Emergency Assistance, Adult Protective Services and Adult Institutional Care.
• Refer clients to Behavioral Health and Health Services or other resource programs as needed.
• Coordinate with agencies to arrange emergency assistance and access to food, clothing, and medical services.
• Supervise Homemaker staff and schedule work assignments.
