Job summary

Health & Human Services Comm in Abilene, TX, is seeking a Community Care Worker I to determine needs and eligibility for CCSE services. You will conduct home visits, interview individuals who are older or have a disability, document information, determine service needs, and develop plans while ensuring accurate authorizations.

Essential duties include interviewing applicants, reviewing eligibility, calculating income, and maintaining case records.

Qualifications

• Excellent written and oral communication.
• Ability to interview applicants and assess needs.
• Experience using Microsoft Office and email.
• Willingness to travel up to 65%.

Responsibilities

• Attends work on a regular and predictable schedule.
• Interviews applicants and conducts home visits to assess needs and provide information/referrals.
• Determines eligibility and processes financial/ service data.
• Maintains case records and enters data in automated systems.
