Job summary

AbsoluteCare in Chicago is seeking a Complex Care Manager (RN) to lead a community pod and manage care for a panel of medically complex members. You will serve as both clinical lead and contributor, guiding CHWs and delivering care plans within value-based care objectives.

The role emphasizes leadership, coordination across teams, and overseeing care plans to close gaps, reduce readmissions, and improve preventive care while honoring patient preferences.

Qualifications

• Licensed RN in the practicing state.
• CCM certification is a plus or must be pursued within 24 months.
• LMSW may be considered as alternative.
• 3+ years of experience serving medically complex populations.

Responsibilities

• Lead community pod as clinical and operational owner.
• Oversee care plans and CHW delegation with accountability.
• Coordinate safe discharge planning and transitions of care.
• Conduct regular care plan oversight visits for assigned members.
• Act as clinical escalation path for CHWs and supervise team.
• Monitor pod KPIs and drive quality improvement.
• Maintain up-to-date EHR documentation within 1 business day.
