Job summary

AbsoluteCare Medical Center & Pharmacy seeks a Community Complex Care Manager in Pittsburgh to lead an integrated care pod and oversee care plans for a panel of members. The CCM provides direct services to the most complex cases and coordinates with CHWs to ensure care continuity.

The role emphasizes value-based outcomes, quarterly care plan oversight, and collaboration with PCP programs. Travel to member homes and clinics is up to 80% with a base salary in the $80–90K range and a sign-on bonus

Qualifications

• Licensed RN in the practicing state and compliant with laws.
• CPR certification and ability to travel up to 80%.
• CCM credential preferred; LMSW may be considered.
• 3+ years serving medically complex populations.
• Experience leading matrixed teams and delegating tasks.

Responsibilities

• Lead community pod as clinical and operational lead.
• Coordinate care for an assigned member panel within a region.
• Provide direct case management for a small panel of complex members.
• Oversee care plans with delegated tasks to CHWs and monitor progress.
• Lead daily huddles, review KPIs, and coordinate population health activities.
• Act as clinical escalation path for CHWs and ensure timely interventions.
