Job summary

Alivio Medical Center Inc is seeking a Care Manager to coordinate care for patients with complex chronic needs. You will screen, assess and develop care plans, monitor progress and support self-management.

The role involves direct patient interaction and telephonic contact, with emphasis on coordinating with primary care providers and care teams. The position requires 3-5 years in care management or disease management, a Bachelor’s degree in a related field, and licensure as a social worker or

Qualifications

• Bachelor’s Degree in social work, nursing or related field.
• Licensed Social Worker or Nurse, or related licensure.
• Demonstrated competencies in EMR usage and patient data management.

Responsibilities

• Identify, track and manage care for high risk patients.
• Conduct initial care coordination screening and CRA/CP.
• Provide ongoing clinical assessments, monitoring and follow up.
• Develop relationships with patients and primary care providers.
• Promote patient activation and self-management support.
• Coordinate inpatient transitions and post-discharge care plans.
• Assist with scheduling follow-up appointments and care coordination.
