Job summary

Sevita, through Mentor Community Services, is seeking a Program Supervisor to lead staff, implement ISP plans, and supervise a team delivering community-based supports to individuals with intellectual and developmental disabilities.

The role emphasizes direct care alongside administrative duties, supervision, and transportation support to help clients participate in daily activities and community life.

Qualifications

• High School diploma or equivalent.
• At least 1 year related work experience.
• Must be 18 years or older; valid driver's license, car registration and auto insurance.

Responsibilities

• Facilitate ISP implementation and participate in staff hiring, training, scheduling and supervision.
• Supervise a team of caregivers supporting individuals in the program.
• Provide support with daily activities, including meal planning, medication administration and personal care.
• Duties include direct support, professional tasks and supervisory responsibilities.
• Facilitate community involvement and transport individuals to work or activities.
