Job summary

Mentor Community Services, part of the Sevita family, delivers community-based supports for individuals with intellectual and developmental disabilities and is seeking a Program Supervisor to lead a team and ensure high-quality services.

The role focuses on implementing ISP, supervising caregivers, and facilitating daily living supports, while coordinating transportation and community activities to promote independence.

Qualifications

• High School diploma or equivalent.
• One year related work experience.
• Must be 18 years or older.
• Current driver's license, car registration, and auto insurance.

Responsibilities

• Facilitate implementation of Individual Service Plans (ISP) and perform administrative duties such as staff hiring, training, scheduling, and oversight.
• Supervise a team of Caregivers supporting individuals in the program.
• Provide support to individuals including daily activities, meal planning, medication administration, and personal care.
• Duties include direct support, program activities, and supervision.
• Assist with community outings and transportation to work or other activities.
