Responsibilities

• Provide coordinated, patient-centered support ensuring timely, effective, and seamless delivery of care.
• Identify and utilize community resources to meet patient/family needs.
• Participate in departmental/organizational committees and promote teamwork.
• Implement the plan developed by the Social Worker addressing patient needs, resources, and goals; educate patient/family on options; prioritize follow-up.
• Promote patient self-management; assist with transportation, scheduling, housing, and food sources.
• Maintain accurate documentation and adhere to policies; comply with HIPAA and confidentiality rules.
