Dorothy Ley Hospice seeks a Community Care Navigator to manage intake, triage referrals, and coordinate access to palliative and bereavement services. The role supports individuals and families in navigating complex health systems with equity-informed care.

You will collaborate with program staff and volunteers, handle EMR data, and help coordinate education and outreach events. This on-site position emphasizes compassionate, person-centered service delivery.

Qualifikationen

• Registered Social Worker with a BSW or equivalent as recognized by the Hospice.
• Active registration in good standing with the applicable regulatory body/college.
• Minimum two years experience with life-limiting illness, caregivers and bereavement.
• Demonstrated triage, problem solving and ambiguity management.
• Expert knowledge of community resources for palliative care, grief and trauma.
• Experience navigating multiple electronic health record systems.
• Commitment to privacy and confidentiality of persons served.
• Excellent interpersonal and communication skills.
• Strong organizational and time management capabilities.
• Past hospice/healthcare/non-profit experience is an asset.

Aufgaben

• Process referrals into EMR and coordinate data collection and communication with referrers.
• Serve as first contact for referrals from healthcare professionals and the public.
• Provide user-friendly information, navigation and informal emotional support.
• Conduct registration and intake screening for referrals (palliative, caregiving, bereavement).
• Offer navigational support to external health and social services.
• Support DLH community team administration, including mailouts, data collection and reporting.
• Respond to in-person needs at the DLH building and participate in Helpline on-call rotation.
• Assist in delivering DLH education and engagement events.
• Coordinate the community volunteer program and support volunteer groups.
