Qualifications

• Active Ohio RN or LPN license.
• BSN preferred or 10+ years hospice LPN experience.
• Minimum 3 years nursing experience, hospice/palliative care preferred.

Responsibilities

• Maintain a living registry of high-risk patients to determine hospice eligibility and palliative care needs.
• Prioritize and manage patient panels based on acuity and urgency.
• Build relationships with PCPs and provide clinically substantive recommendations.
• Lead goals-of-care conversations with patients and families.
• Coordinate advance care planning and ensure documentation.
• Own day-to-day relationships with hospice and palliative care agencies.
• Document assessments and CMS/compliance information in medical records.
