Job summary

The Jewish Board of Family and Children’s Services seeks a Care Coordinator to link adults and children with chronic behavioral health and medical conditions to needed services. The role involves field-based work, risk assessment, care plan development, and coordinating care across health and community services.

Responsibilities include arranging appointments, educating members, tracking referrals, and ensuring access to required services as defined in each ICP.

Qualifications

• Bachelor's degree in a relevant field and NYS licensure or certification requirements may apply.
• Experience in care coordination or care management in medical or behavioral health settings.
• Fluency in a second language such as Spanish, Russian, or Creole is preferred.

Responsibilities

• Integrate medical, behavioral health, and social support services for clients served.
• Periodically assess medical/behavioral health needs and treatment compliance.
• Collaborate to develop an ICP with the member, family, and providers.
• Provide required care management services and coordinate appointments.
• Document all activities and ensure access to ICP-defined services.

Bachelor's degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing

NYS teacher certification or RN licensure with bachelor's degree + experience in direct services/care coordination

Bachelor’s/Associates/High School Diploma + five years' applicable population experience
