Area Substance Abuse Council in Cedar Rapids is seeking a full-time Care Navigator to provide care coordination and supportive services for Linn County patients. You will link patients to services and help overcome barriers to long-term recovery, working with clinical staff and community resources.

As a full-time Care Navigator for Linn County, your role will be to provide care coordination and supportive services to patients in the area. You'll utilize an intensive case management model to link patients to services such as childcare, transportation, housing, employment, and education in order to help patients overcome barriers to their long term recovery.

ASAC offers a competitive compensation with a starting wage of $22.71 Additional compensation is available for candidates with extensive experience.
