• Formal training in behavioral health, typically a Bachelor’s Degree in a related field.
• Valid state-issued driver’s license is required.
• Proficiency with MS Word, Excel and basic presentations; experience with EHR and scheduling systems.

Responsabilidades

• Screening, assessment, and engagement of individuals and families; identify risk factors and ensure timely intervention.
• Coordinate care across healthcare, behavioral health, housing, employment, and social services.
• Maintain timely, accurate documentation in the EHR and participate in treatment team meetings.
• Advocate for access to services and coordinate transportation or scheduling to reduce barriers to care.
