Job summary

Prestera Center for Mental Health Services in Charleston, WV is seeking a dedicated Case Manager to coordinate services for consumers, identify needs, advocate, and link resources while maintaining crisis services when needed.

You will conduct psychosocial assessments, develop service plans with the treatment team, provide supportive interventions, review progress, and maintain records in accordance with policies and HIPAA.

Qualifications

• Experience or passion to work population served.
• Valid driver's license required.
• Must have strong written and verbal communication skills and good time management.

Responsibilities

• Conducts interviews with consumers and their legal guardians to collect information for psychosocial assessments.
• Develops service/treatment plans with the treatment team within identified timeframes.
• Provides supportive intervention services to consumers and assists with resources.
• Schedules reviews of service/treatment plans and monitors consumer progress.
• Assesses and reassesses consumer needs using screening and assessment tools.
• Reviews treatment goals and collaborates with teammates.
• Provides crisis assessment and develops safety plans as needed.
• Maintains case records, reports, and documentation per guidelines.
