Prestera Center for Mental Health Services in Logan, WV seeks a dedicated behavioral health professional to perform screening, assessment, and care coordination within community settings.

You will identify risk factors, utilize PHQ-9 and other tools, coordinate services across health, housing, and social supports, arrange transportation, and participate in treatment teams; maintain accurate EHR documentation.

Qualifications

• Formal training in behavioral health, usually in Psychology, Social Work, Counseling, or Criminal Justice as part of a Bachelor’s Degree program.
• Maintains current knowledge of standards of care through continuing education.
• Valid state-issued driver’s license required.
• Proficient with MS Word, Excel, PowerPoint; experienced with EHR systems.
• Strong organizational and communication skills; able to work in community settings.

Responsabilités

• Performs screening, assessment, and engagement with individuals and families.
• Identifies risk factors including suicidal/homicidal ideation and follows protocols.
• Coordinates services across healthcare, housing, and social systems; participates in treatment teams.
• Schedules appointments, assists with transportation, and maintains compliant documentation in EHR.
• Advocates for clients to access needed services and resources.
