Job summary

COMMUNITY CARE OF WEST VIRGINIA INC in Weston, WV is seeking an Intake Coordinator to conduct initial assessments for behavioral health referrals and triage to appropriate providers. You will collaborate with referral specialists and care navigators to round out the BH referral team and ensure efficient patient processing.

The role requires a graduate degree and prior outpatient therapy experience, with duties spanning patient communication, waitlist management, and policy adherence.

Qualifications

• Masters degree required.
• Two years’ experience providing therapy in an outpatient setting.
• Licensed independent therapist (LICSW, LPC or equivalent).

Responsibilities

• Conducts initial assessments for behavioral health referrals.
• Assist referral specialist and care navigator with initial contact of referrals.
• Maintain BH services waitlist and ensure timely communications.
