Job summary

Summit Hospital in Show Low, AZ is seeking a Community Resource Coordinator to support high-risk patients post-discharge and reduce readmissions. You will work within a multidisciplinary team to coordinate psychosocial resources and options across health and social care settings.

Responsibilities include psychosocial assessments, care planning, resource coordination, and liaison with Behavioral Health services, with emphasis on maintaining patient rights and documentation in the EMR.

Qualifications

• Undergraduate degree in Social Work or related field.
• 1+ years of related experience in crisis intervention, medical social work, and/or community case management.
• BLS/CPR certification within 30 days of hire.

Responsibilities

• Performs psychosocial assessments to identify patient and family needs.
• Adds to multidisciplinary care plans with patient input and mutual goals.
• Uses counseling, motivational interviewing and education to address issues.
• Aids in securing resources for financial, transportation, housing needs.
• Liaises with local Behavioral Health services and resources.
• Case manages patients with behavioral health or substance use diagnoses.
• Develops relationships with primary care and hospital networks for care management.
• Maintains documentation in the electronic medical record.
• Respects patient rights and confidentiality; upholds professional integrity.
• Monitors outcomes and suggests process improvements.
