Job summary

The Community Support Professional (CSP) helps individuals achieve greater independence by delivering person-centered support across housing, healthcare, employment, and personal development, aiming for full community integration.

You will engage with clients, coordinate referrals, assist with entitlements, monitor care plans, and build collaborative relationships with families and providers to promote stability and growth.

Qualifications

• High school diploma or GED required.
• Bachelor’s degree preferred in Human Services, Social Work, Psychology, or related field.
• Minimum 2 years of experience in homecare or community-based services.
• Strong communication, documentation, and organizational skills.
• Valid driver’s license and reliable transportation preferred.

Responsibilities

• Establish and maintain professional rapport with clients, upholding ethical boundaries.
• Support client integration by connecting with local resources for housing, healthcare, and social inclusion.
• Maintain regular contact with clients as defined in their service plan.
• Coordinate referrals for medical care, entitlements, transportation, education, employment, legal assistance, and more.
• Assist with money management and entitlements.
• Develop, implement, and monitor person-centered care plans from intake to discharge.
• Enhance independent living skills through instruction and hands-on assistance.
• Identify barriers to clients’ goals and connect them to services addressing challenges.
• Conduct regular meetings to review progress, mental health, appointments, and discharge readiness.
• Perform routine apartment or room inspections and assist with maintaining cleanliness as needed.
• Build collaborative relationships with families, natural supports, and external providers.
• Attend team meetings, trainings, supervision, and rounds.
• Support clients with completing redetermination forms, SNAP applications, and other essential documents.
• Submit encounter notes within 24 hours of client interactions.
• Submit monthly summary reports by the 21st of each month.
