Job summary

Coleman Health Services seeks a Manager for OhioRISE Care Coordination/Case Management based in Mansfield to lead a Care Coordination team driving wraparound services for youth and families. You will shape service plans, supervise coordinators, and partner with child-serving systems to improve outcomes.

Ideal candidates hold an LSW/LPC or higher, have at least 3 years of relevant experience, and can manage complex cases while ensuring timely documentation.

Qualifications

• Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or higher.
• Demonstrated supervisory or managerial experience in community-based services.
• Minimum 3 years in children's mental health or related public sectors.
• Experience managing complex cases involving multi-system youth.
• Monitor documentation quality and coach staff to performance.
• Maintain an outstanding driving record.
• Ability to obtain NPI and Ohio Medicaid numbers (assistance available).

Responsibilities

• Provide strategic oversight for structured service planning and care coordination.
• Develop and supervise a team of Care Coordinators.
• Engage with child-serving systems and behavioral health providers.
• Initiate quality improvement initiatives aligned with OhioRISE requirements.
• Support development of the local system of care serving youth and families.
