Job summary

CareSource seeks a Community Based Care Manager to collaborate with an interdisciplinary team to coordinate services for OhioRISE members and related programs. The role involves comprehensive assessments, culturally competent care planning, and close member engagement in diverse settings.

Requirements include a nursing or health-care degree and current clinical licensure in Ohio; 3+ years experience in nursing, social work, or related fields.

Qualifications

• Nursing degree or Bachelor’s in health care field required.
• Advanced clinical licensure preferred; RN/LSW/LISW/LCSW/LPC/LPCC license in Ohio required.
• Three+ years in nursing, social work, counseling, or related health care role.
• Strong knowledge of quality, HEDIS, and disease management.

Responsibilities

• Engage with members and natural supports using strength-based assessments.
• Develop ICP with the ICT based on member needs and preferences.
• Identify and address barriers to ICP goals and implement interventions.
• Coordinate with ICT and providers to ensure seamless post-discharge planning.
• Educate members and caregivers about treatment options and resources.
• Document care coordination activities per policy and practice standards.
