Qualifications

• Bachelor's degree in behavioral health or 2 years case management in behavioral health, or CHW certificate.
• Ability to conduct screenings, assessments, and follow-ups with identified patients.
• Knowledge of prenatal care systems and postpartum services.
• Must have valid Texas driver's license and reliable transportation.

Responsibilities

• Represent the program at community meetings and know local resources for referrals.
• Maintain program supplies and coordinate approvals and delivery of supplies.
• Coordinate non-medical basic needs support and timely delivery with documentation.
• Assist in monthly programmatic reporting with all staff activities and demographics.
• Facilitate program activities including outreach, intake, referrals, classes, and follow-up.
• Ensure outreach materials are organized, updated, and culturally appropriate.
