Job summary

The Texas Health and Human Services Commission (HHSC) in Houston is seeking a Community Care Worker III to support Medicaid waiver programs, enrollment, and member services within STAR Kids MDCP and STAR+PLUS HCBS initiatives.

You will collaborate with MCOs and HHSC teams to determine eligibility, process enrollments, and assist with transitions from nursing facilities to community living, with telework options and comprehensive benefits.

Qualifications

• High School Diploma or GED required.
• Prefer experience with case management.

Responsibilities

• Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
• Develop, coordinate, and explain waiver programs to applicants and members. Authorize or deny waiver services, send approval and denial notices. Validate eligibility in appropriate systems.
• Register waiver program eligibility in authorization system and coordinate with MEPD staff for enrollment in Medicaid.
• Assist denied Medicaid waiver members to file a fair hearing and track processes through to decision.
• Handle reports for monitoring program operations and resolve enrollment errors.
