Job summary

The Texas Health and Human Services Commission (HHSC) in Arlington, TX is seeking a Community Care Worker IV to handle advanced case management for STAR Kids and MDCP waivers, helping clients select providers and coordinating transitions from nursing facilities back into the community.

You will work with MCOs and internal HHSC teams, utilize TIERS and related portals, verify eligibility, and assist with fair hearings while guiding staff and supervisors as needed.

Qualifications

• Advanced case management for STAR Kids and MDCP waivers.
• Experience with TIERS, Long-term Care Portal, and MCO enrollment.
• Ability to coordinate transitions from nursing homes to the community.
• Supervisory duties and training of staff when acting supervisor.
• Strong communication with providers, members, and program staff.

Responsibilities

• Develop STAR Kids/MDCP waiver authorizations and notices.
• Coordinate transitions from nursing homes to the community.
• Monitor contract agency services and resolve delays.
• Assist in MDCP interest list management and fair hearings.
• Prepare management reports and monitor enrollment issues.
• Provide training and policy guidance to staff.
